2025-2026 Identity and Statement of Educational Purpose Form

Print the attached 2025-2026 Identity and Statement of Purpose form. You will need to complete and sign it in the presence of someone in the VCSU Financial Aid Office, located in McFarland 219, or in front of a Notary if you are unable to appear in person at VCSU. You will also need to show a valid government-issued ID, such as a driver's license, to us or the Notary. If you have this form signed by a Notary, you must also provide us with a copy of your id.
