The Personnel Action Form (PAF) should be completed for any VCSU employee with employment and classification changes and/or payroll changes.

*Note I, there may be times that not all sections need to be completed. For example, if you

have an employee receiving a one-time pay, you will need to complete Section I, III and IV.


*Note II, HR/Payroll will route this form to the appropriate personnel you provide for

approval.


*Note III, if you have multiple additional pay submissions, please enter “Multiple” in the

employee name, “Zero” in employee ID, and attached the excel spreadsheet, not in PDF

format.
