VCSU Consortium Agreement
Consortium Agreement Information A Consortium Agreement is a contract between two colleges/universities that recognizes your registration at each location for financial aid purposes. It also certifies only one of the two institutions can administer Title IV and state financial aid. Student who enroll in courses at other institutions (other than NDUS campuses) are required to complete the “Consortium Agreement". These credits are considered part of their term/semester load. Please note the “General Requirements and Notices” section explains the steps student will need to follow each semester.

Financial Aid Satisfactory Academic Progress Appeal Form
In order to appeal your financial aid suspension, complete all information on attached form and return this form along with any supporting documentation and your advisor’s approval. Appeals must be submitted by the end of the third day of class for the appealed semester. All institutional charges from prior semester must be paid in full before appeals will be considered. The Financial Aid Committee will review your appeal based on the Satisfactory Academic Progress Policy found at: (https://onestop.vcsu.edu/support/solutions/articles/10000034255-undergraduate-satisfactory-progress-policy). Submitting an appeal does not guarantee approval to reinstate eligibility to receive federal financial aid. The committee will make a final decision, either approving or denying your appeal. You will be informed of their decision by letter and email to your VCSU email account. If the appeal is not successful, students may consider an alternative/private loan to fund their cost of education until they meet the minimum SAP standards. Students are responsible for paying university charges incurred and will not be eligible for federal financial aid unless this appeal is approved. Undergraduate Previous Bachelor's Degree Form
You have indicated on your FAFSA (Free Application for Federal Student Aid) information that you have a bachelor's degree. To be eligible for Federal Student Aid, a student with a prior bachelor's degree must be enrolled in an academic program that is degree-seeking and have a program plan and Undergraduate Previous Bachelor's Degree form on file. Print, complete, and sign the attached Undergraduate Previous Bachelors Degree form. 3-Year Completion Form
This form (attached below) is to request that renewable scholarships be utilized during the summer terms for students who intend to complete their program in three years. STIPULATIONS: This agreement only affects renewable scholarship funds (President’s and Leadership) awarded by Enrollment Services as an incoming freshman. Summer awards cannot exceed the amount of tuition and fees for the term. Student may change plans and attend more semesters; however, that student will only be eligible for the maximum initial amount awarded. Student will determine their 3-year path during their 2nd semester of enrollment. Please return the completed form and your Plan of Study to the Office of Academic Affairs, McFarland 213, during business hours. *A Plan of Study is a document that identifies the requirements for a specific program at a post-secondary institution and how the student will fulfill them. This document may also be called a Degree Audit Report or Academic Advisement Report and is set up with the help and approval of your advisor.
