Name Change Form
All Legal Name Change Request eForms must be accompanied by the 2 following documents:
1. Legal documentation showing the name change. Acceptable forms of documentation: birth certificate, marriage certificate, government-issued identification card or driver’s license, passport, or court order.
2. A signed social security card reflecting the new name. For non-U.S. citizens, an updated passport can be used in place of a signed social security card.
This request will change the primary and preferred name on the student academic record.
To submit a Legal Name Change Request, SSN card and legal documentation, do the following:
1. Log into Campus Connection
2. Click on the VCSU eForms square
3. Click on Records Office on the left panel
4. Click on Legal Name Change Request on the left panel
5. Complete the form, attach your SSN card and one of the approved legal documents listed above
6. Click Submit
Once the eForm is submitted, the request will go to the Office of the Registrar to update your name in Campus Connection and you will receive an email to notify you when the eForm has been processed or if additional information is needed.
AFTER a legal name change has been entered in Campus Connection, students will still need to reach out to Technology Services to change their NDUS dotted identifier (NDUS ID). The process to rename a user’s NDUS ID begins once the local help desk changes the NDUS ID in the DNA system, not Campus Connection.
