Name Change Instructions for Staff, Faculty, & Student Employees
Please complete the following steps in numerical order (1-6) to change your name. HR/Payroll Office: Complete a new Form I-9 online. Go to Employee Self Service>Personal Details>Form I9 and bring your new acceptable identification documents to the Human Resources/Payroll office at McFarland 211. A social security card is required. One document from List A or B is also required. Notify HR/Payroll if you wish to use a Notary Public as a designated agent in place of completing the form in person. Employee Tuition Assistance The North Dakota State Board of Higher Education encourages its employees to pursue a program of continuing education. To qualify for this benefit, the employee and their courses must meet the criteria outlined in SBHE Policy 820. Employee Tuition Waiver Form Instructions Spouse/Dependent Tuition Assistance The North Dakota State Board of Higher Education allows campuses to adopt tuition waivers which are consistent with an institution's mission. To qualify for this benefit, the employee, dependents, and their courses must meet the criteria outlined in SBHE Policy 820 and VCSU Policy V820.01 Tuition Discount – Spouse and Dependents. Verification of Spouse/Dependent is required to be on file to process a tuition discount. Please submit your verification to the Office of Human Resources (McFarland 211) with your first spouse/dependent form.
