5. Choose the type of student you are: Undergraduate or Graduate.
6. Choose the term this petition applies to.
7. Use the magnifying glass by the advisor look up. Only put in the first OR last name, then search.

8. Choose the purpose of your petition using the drop-down menu.
9. Use the magnifying glass by the look up. 

1. If you have a class number, ONLY put that in (e.g. 5473), then search.
2. If not, ONLY put in the Subject Area (e.g. HPER) and Catalog Number (e.g. 307), then search.
3. Choose the class. Double check that you have the right one.

10. Use the magnifying glass by the instructor look up. Only put in the first OR last name, then search.
